Possible reasons:

It doesn't have any content that can be indexed - for example PDF file that have scanned images of some text, but no text that can be indexed.
Password protection on the documents you upload.

Indexation of Japanese, Chinese, Korean, and other multi-byte languages

For correct indexation of an attachment in Japanese, Chinese, Korean, and other multi-byte languages, the attachment must be encoded in UTF-8 encoding. Other encodings are not supported.

General Settings gets frozen and shows infinite loader

The General Settings page is no longer loading. It just shows a message saying "loading..." but never comes up. It is most likely that you've installed attachments indexation plugins recently, but something went wrong. This guide will help you to troubleshoot this problem.

When you go to Administration > General Settings, KMP launches a test for each attachment indexation plugin to find out which is present and working correctly, and which is not. If one of the plugins will get frozen, it will also freeze the General Settings page.

Please check permissions to the cmd.exe (it is located in C:\WINDOWS\SYSTEM32\ folder) and all attachment indexation plugins (they are located in \admin\application\libraries\search\plugins\ folder of KMP). If your server runs under IIS, you should grant the Internet Guest Account, NETWORK SERVICE, or IUSR_SERVERNAME (depending from IIS settings) with the read and execute permissions to all these files. If it is Apache server, you should check which user runs the Apache process and grant read and execute permissions for that user.
Windows systems often has security issues. For example when you download and exe or dll file, it gets blocked "for security reasons". In this case, you need to open properties of each file (right click > properties) and click on the "unblock" button.
We strongly recommend you to update to the latest version of PHP. These plugins work under PHP v5.3+ correctly. Earlier versions of PHP have bugs and may freeze when launch external programs (e.g. attachment indexation plugins) using Windows command line.

Indexation of small (<12Kb) DOC files does not work

KMP uses free AntiWord tool for indexation of DOC files. This tool has certain limitations that doesn't allow it to index attachments created in Word for Mac and OpenOffice that are lesser than 12Kb.

To avoid this issue we recommend to use DOC files created in Windows version of Word, or use files with more than 1024 bytes of text inside (which is equivalent to 12Kb of total file size) if they are created in Mac version of Word or OpenOffice .

This is not a bug, it is a missing feature.

Let me explain.
Inside a Word file the text is stored in a so called text stream. There are
two possible text streams: a small block text stream and a large block text
stream. The small blocks are 64 bytes in size, the large blocks are 512
bytes in size. Because the difference in size Antiword would need two
different methods for reading those two text streams. The method for
reading that small block text stream has not been implemented yet. The
result is that Word files with no large block text stream can no be read by
Antiword. Such Word file are mostly smaller than about 12 kilobytes and
have less than 1024 bytes of text.

The reason for not implementing the missing fearture is simple. Word
documents that use the small block text stream can not be produced by Word
for Windows (all versions), but only Word for Mac. And now by OpenOffice.
Note that these documents can be read by all versions of Word.
